Has my video been illegally downloaded?

No, it hasn't. The video that appears on the page has been shared directly from your YouTube channel using the official YouTube video insertion tool and no download or other illegal action has been performed.

How do I check that the video that appears here is the same as my YouTube channel?

The easy way is to just click in the YouTube's logo button in bottom right corner of YouTube's player. This open a new browser tab of the video in YouTube. Then you can check that the video is directly from your channel.
Also, if you access the Creator Studio dashboard of your channel and then the Playback Locations of Analytics, you'll see a list of pages where your videos has been successfully inserted. If you have selected your video you will be able to know how many reproductions come from this page. If it were not your video, no playback would appear. Note that statistics takes two days to update.

Is it harming me that my video appears here?

Of course not. Adequate precautions have been taken to ensure that the owners of the videos are not harmed and taken care of so that both the monetization for advertisements and the record of reproductions are working completely and benefit the owner of the video.
